TERMS OF ENGAGEMENT LETTER - Between the BANT Nutritional Therapist and the Client

What is Nutritional Therapy? 
Nutritional Therapy involves the application of nutrition science in the promotion of health, peak performance and individual care. A wide range of tools may be used to assess and identify potential nutritional imbalances and it will be explained how these may contribute to your individual symptoms and health concerns. This means that my recommendations will be made to address nutritional balance and help support your body towards maintaining health. 

Acknowledgement of my scope of service 
The degree of benefit obtainable from Nutritional Therapy may vary between clients with similar health problems and following a similar Nutritional Therapy programme.
My nutritional advice will be tailored to support you as an individual and will be agreed between us.
Nutritional Therapists are not permitted to diagnose, or claim to treat, medical conditions.
Nutritional advice is not a substitute for professional medical advice and/or treatment.
I may recommend food supplements and/or functional testing as part of your Nutritional Therapy programme and I may receive a commission on these products or services.
Standards of professional practice in Nutritional Therapy are governed by the Complementary and Natural Healthcare Council (CNHC). Code of Conduct.
This document only covers the practice of Nutritional Therapy within this consultation, and it will be made clear to you if I intend to step outside this boundary.

I request that you understand and agree to the following terms:
You are responsible for contacting your GP about any health concerns. 
You give me permission to contact your GP regarding any agreed aspects of your case. 
If you are receiving treatment from your GP, or any other medical provider, you should inform him/her about any nutritional strategy provided by me. 
It is important that you tell me about any medical diagnosis, medication, herbal medicine, or food supplements, you are taking as this may affect the nutritional programme.
If you are unclear about the agreed Nutritional Therapy programme / food supplement doses / time period, you should contact me promptly for clarification.
You must contact me should you wish to continue any specified supplement programme for longer than the original agreed period, to avoid any potential adverse reactions and to ensure the best programme has been chosen for your needs.
Recording consultations using any form of electronic media is not allowed without the written permission of both you and me.

Fees
My professional fee structure is as follows: £95 for an initial consultation of 1½ hours; £50 for a follow up of 45 mins. These charges include brief email or phone contact between consultations if required.

Emails and phone calls that take longer than five minutes and any letters will be charged at on a pro rata basis of £60 an hour – I’ll invoice you when it gets to half an hour of work.   

This fee structure exists so that you can receive my full attention, which requires reviewing the entire file prior to a conversation, for example, and note making after the conversation or appointment. This is similar to and in line with the fee structure of most other professionals. 

Cancellations can be made up to 24 hours before the consultation. After that time any cancelled appointment will be charged at the full rate.
